This year, made a slight change with regards to the questionnaire format:

• New formats available in both MS Word and Excel
• Planning/ Objectives and Achievement are summarized under each core subject, i.e. no need to fill in planning/ objective and achievement under each practice
  
• International standard and certification award can be listed
